We Welcome New Patients

As a patient, you will have the opportunity to meet with our team and get to know our endodontic specialist, Dr. Michele Scrime. We are committed to educating you about your dental needs and will answer any questions you may have concerning your treatment.

Our goal is to provide you with the highest level of endodontic care available today. Thank you for your confidence in our office and we look forward to meeting you.


Please arrive 15 minutes prior to your appointment time and bring the following:

  • Photo ID (valid drivers license or other)
  • Dental Insurance Card (or downloaded copy from insurance company website)
  • Referral form from your general dentist or referring doctor
  • Current X-rays of the problem area (unless already sent to us by your dentist)
  • Completed New Patient Forms (Health History, Registration, HIPPAA)

New Patient Forms

To provide you with the most efficient new patient experience, please download and complete the forms below and bring them to your first appointment. Our team will take comprehensive records and address any concerns or questions you may have. If you have any problems downloading the forms, please contact our office and we will gladly send them to you.

  • Download & Save the Forms (please save as PDF)
  • Fill out the requested information on each form and save a copy to your computer.
  • Attach and send your filled-out forms to our office email at jsendo@cmonds.com

Your First Visit

Your first visit with us may include:

  • Complete evaluation of your dental problem
  • Digital X-rays
  • 3D Cone Beam Scan (if required)
  • One-on-one consultation with our endodontist to discuss your needs, concerns, goals, and treatment options


We are a FEE FOR SERVICE OFFICE. Payment is expected at the time of service. We accept all forms of payment including check, cash, credit cards (MasterCard, VISA, Discover, and American Express)

Insurance and Billing

We recommend that you contact our office prior to your appointment to provide us with insurance information for verification of benefits. Our practice is a participating in-network provider with Horizon Blue Cross Blue Shield (Traditional only) and Delta Dental Plan (Premier). We are considered an out of network office with most other major dental insurance companies. We will file claims for you as a courtesy. For these plans, payment in full is expected at the time of service and you will be reimbursed directly from your out of network insurance company. The amount of reimbursement is based on your plan’s benefits and may not be equal to your total fee. Whenever possible, we will provide you with a financial estimate prior to any treatment rendered, and work with you to obtain financing if needed.

Financing Options

We understand that endodontic treatment is an investment in improving the quality of your life. To make the total cost of treatment more affordable on any budget, we offer CareCredit℠ patient financing. This program works just like a credit card that is available for medical and dental procedures, and has low interest rates with flexible repayment terms. Our administrative team will be happy to assist you with any insurance or financing information.

We work with Care Credit Health Care Financing to help our patients obtain the treatment they need by extending payments to fit many budgets.

Please understand that your insurance is a contract between your employer and a dental insurance company. Benefits received are based on the terms of the contract negotiated between your employer and the dental insurance company and not our dental office.

The goal of most dental insurance policies is to provide only basic care for specific dental services. The benefits that you receive from the dental insurance company for your current insurance plan have nothing to do with you achieving a high-quality, complete result.

Your benefits are decided between your employer and the insurance company. Many needed services may not be covered. Please remember that you are ultimately responsible for all expenses incurred. We urge you to read the policy so that you are fully aware of coverage and any limitations of the benefits provided. Our staff will be happy to assist you with any insurance matter. Please address any questions regarding Financial Options, insurance, and fees to our Patient Financial Coordinator.
